This is because Ashwagandha can – depending on the dosage and individual disposition – have neuromodulatory, hormonal, and immunological effects. Caution is particularly advised when taking medication.

    Ashwagandha and St. John's Wort – Risk of Serotonin Excess

    Both Ashwagandha and St. John's Wort can interfere with the central nervous system – especially with serotonin metabolism. Simultaneous intake can increase the risk of serotonin syndrome, a rare but potentially dangerous overstimulation due to too much serotonin in the body.

    Recommendation: This combination should only be used under medical supervision – or avoided if possible.

    Ashwagandha and Antidepressants – particularly sensitive combination

    Ashwagandha acts on the central nervous system – in particular by modulating GABA, cortisol and possibly serotonin. These very systems are also target structures for many antidepressants:

    • SSRIs (e.g. fluoxetine, sertraline)
    • Tricyclics (e.g. amitriptyline)
    • MAO inhibitors (rare)

    Important note: When taken simultaneously, unwanted interactions, potentiation or weakening of effects or unexpected side effects may occur. Changes in emotional processing are also possible.

    Ashwagandha and thyroid medication – possible influence on hormone levels

    Ashwagandha can influence the levels of thyroid hormones T3 and T4 – particularly in people with latent or manifest hyperthyroidism, this can become critical. Anyone taking medication such as L-thyroxine or other thyroid hormones should discuss a combination with Ashwagandha very carefully with their doctor.

    Possible effects:

    • Increased effect of thyroid medication
    • Irregular heartbeat, nervousness, sweating
    • Caution is particularly advised for autoimmune thyroid disease (Hashimoto).
